View personal data (name and date of birth)

 

The personal data section displays your name and date of birth as entered in your payroll record.  This information is read-only. Please complete the Change of Title and Name Form and forward this along with any supporting documents required to [email protected]

 

Click the display icon ( ) in the Personal Data section to view the information

       

 After viewing, click  to return to the previous screen.
